For the second time in two years my Miata has thrown the alterator adjusting block and bolt.

I replaced it after the first time with a Mazda replacement and new bolts. Tightened the adjuster bolt and tensioned the belt then locked everything down with lock washers. I have been checking it periodically with no signs of loosening up. Then last night, about a year after the repair, I heard what I thought was something I ran over. About a half hour later my headlights became dim and my ABS light came on. The Alternator light never came on and the car did not show an increase in temperature. As I got off the free way at my exit the car began to misfire. I shut off the lights using only the running lights to make it home. Upon inspection the lock down bolt and adjusting bolt and block were missing, just like last time. The belt was still in place apparently keeping the water pump tuning fast enough to keep the car cool but not fast enough to charge the battery.
Has anyone else experienced problems with this adjusting bolt and block. I ordered the new parts and they should be here Friday. For now I used the old school method of a pry bar and a locking bolt to keep the alternator belt tight. Any input would be appreciated.
